Key Components of Substance Use Case Management

Substance Use Case Management (SUCM) is a critical support service provided within Virginia’s behavioral health system, particularly as part of the Addiction and Recovery Treatment Services (ARTS) program under Medicaid, and is overseen by the Department of Medical Assistance Services (DMAS).

This service is designed to support individuals with substance use disorders (SUD) by helping them navigate treatment systems, access essential resources, and maintain engagement in care, especially when managing complex social, health, or behavioral needs.

Core Functions:

Case managers assist individuals by:

  • Assessing client needs (clinical, housing, employment, medical, legal).
  • Developing and monitoring individualized service plans.
  • Coordinating care across service providers (e.g., SUD treatment, mental health, primary care).
  • Linking clients to services including MAT, detox, residential programs, social services, and recovery supports.
  • Advocating for client access to services and rights.
  • Monitoring progress and adjusting plans as needed.
  • Supporting transitions between levels of care (e.g., from residential to outpatient).

Eligibility:

Individuals may qualify for SUCM if they:

  • Are diagnosed with a substance use disorder (mild, moderate, or severe).
  • Require assistance with accessing, coordinating, or sustaining treatment and recovery services.
  • Face barriers to independent functioning due to substance use, such as housing instability, legal involvement, or co-occurring mental health needs.

Service Setting:

  • Delivered in a community-based setting: outpatient clinics, residential programs, recovery centers, or through home/community visits.
  • Case management is non-clinical, but tightly integrated with clinical services.
